A review meeting can be conducted immediately by the Trustees and concern should be expressed about poor performance of students to all the concerned teachers. The academic planning must be reviewed in presence of Head. The reasons of poor performance must be delibrated at length and serious measures must be chalked out and the Head be asked to monitor.
With dignity, Reprimand/Mild firing to the Head when she is alone with to improve the poor academic performance is extremely necessary.

It depends on the circumstances. If heads are required in their job descriptions to ensure students perform well at school, then their performance would judged as such. But if they are not, then it would not be fair to fire them.
There is no need to fire the head. The school only need to review re-visit and look for better strategy to improve on the performace of the students. Such review could lead to staff development through training, workshops and others

No. I don't think it is an appropriate action. Although the School Head is responsible for the academic record, a collective effort from teachers, mentors and parents will substantially improve the performance of the students. Regular meetings with teaching staff and in-depth analysis of the student records will further help in identifying the loopholes.
