Why do many guidelines still state that ranitidine is to be avoided during breastfeeding (because it passes through breast milk) even though ranitidine is used itself in children over 1 month of age (and the amount passing through breast milk is thought to be significantly lower than that given to childrens directly) ? Does this recommendation make any sense ?

There are studies of ranitidine in infants both via mothers milk and directly that show minimal risk. The majority of drugs are not tested in pregnant or breastfeeding women. To exclude all drugs from use because they are not tested in the specific population is not feasible. Biochemical pathways and animal studies that indicate low risk are often used as guidance.
