Dear. Your question have some assumption. You have assumed that the soul transition from one material body to another one is a true and real phenomena. Some of religious confirm that and most of them denied it. The main soul transition which many of people believe it, is soul transition from the material body to the another world which is different from this physical world.

Also you assumed that this phenomena contains some quantum properties which may be able to detect via quantum measurement.

I think that physics and QM are too far away to have any comment about such these questions. But quantum mechanics due to its formalism relation to observer and some important physical properties such as nonlocality and entanglement may be has some comments about the soul and brain relation.

I think that if QM can have any comment is related to the interaction between soul and body not about the soul.

That is true.. but I am proposing a hypothesis relating soul transition from one body to another body and wants to examine its certainties through quantum mechanically. Let's see up to what extent the hypothesis fulfill.

No, since it's not known how to define what ``soul'' means in this context. And whatever definition is proposed can't have anything to do with quantum mechanics, since it must make sense for classical objects, which can be identified as having a soul, namely human beings.

I don't think the quantum mechanics is able to describe souls and their transitions. Those are two completely different worlds.
