I am looking for inernational examples of online- or paper-pencil-questionnaires regarding the employees' trips to work and back as well as business trips. These questionnaires should be standardised (offering a certain set of questions / variables) and available for any company or institution, interested in surveying the mobility behaviour of it's employees.

This is very interesting for anyone who deals with the movement of people. Does your observation includes all categories eg. and people with disabilities. I am also interested in this category and how you dealt with and their movements it would be nice to see this results also.

I am not sure if I understand your question. My question and the answer of Patrick Hoenninger target the standardised surveying of trips to work and business trips. The links (in German language) refer to such examples.
I am interested in the existence of a standardized toll for surveying and (data) analysis of such mobility surveys.
