Anyone have an example of a survey that asked multiple content aspects with multiple hours attended in the one question?
I want to ask my sample of health workers if they have attended training on a topic that including knowledge only lectures, skills and application of theory (classroom), and/or practical training in using the knowledge and skills (real situations) and also if this training was less than 3 hours, 3-7 hours, 1-3 days etc duration.
I cannot find a clear way to format this question (for Survey Monkey). For example if the respondent attended 7 hours of training that included theory, application and practice within that 7 hours and they tick the three categories it is coming out as if they attended 7 hours of each aspect.

I recommend using a "branching" type item structure with only one concept in each "branch."
For example, "Did you receive training on "x" topic in the last (period of time). Yes/No
If "no," skip to item xxx (or end survey if you only want any further information from those who did participate in some training.)
If, "yes,"
a - how many hours of the training consisted of classroom acquisition of knowledge and/or skills
- less than 3 hours
- between 3 hours and 7 hours
- more than 7 hours
- training did not include this component
b - how many hours of the training consisted of classroom application of knowledge and/or skills
- responses as above
c - how many hours of the training consisted of application of knowledge and/or skills in actual clinical settings
- responses as above
You can then set the data analysis parameters to get the information you want broken out the way you want -- hours by type
Typically, it is best to restrict each survey item to a single concept.
In order to capture all of the data you want in one question, the response options would have to include every variation, which would in turn potentially cause respondents to not finish the survey because they would need to sort through numerous options to find the one that. reflects their experience.

Karen is exactly right. Follow her direction. One never asks a subject to respond to more than one topic in a single survey question. Impossible to give a single objective response when two responses are required.
Adding to this: You need to think in variables and values, not only the research questions You want to address. Listing as variables with values, makes the structure Karen propose.
Happily, we now have databased surveys, that will pipeline the respondents whereever their responses lead, reducing the N/A-values.
I suggest you can't do this in one question. What could be done is to use the branching type questions as suggested by Karen. The first part is to clarify which parts were attended, then to move to exclude those which are not appropriate. Then to focus on the impact of those parts that are appropriate. Unfortunatley, Survey Monkey does not do this, you need to design your own survey with something like Qualtrics which will allow more selective responses. It is also important to recognize that you will be getting their perceptions of the impact. It is also possible to do focus groups with a semi-structured process to get additional information that the formal survey will miss.
I recommend brea the question down into time spent and the content of the time. Break the multiple aspects of content into separate items. Followping this procedure will make data analyses much easier.
