I believe that guidelines are meant to be used for guidance only, and not as laws. Most guidelines nowadays give various options and conclude by advising an individualized management protocol, I personally appreciate all such guidelines.
I think protocol based knowledge and clinical knowledge both are different things. After few years of clinical experience, everyone starts working on their own.
I think both guideline-protocol-algorithm and personalized clinical judgment come one after another. When you are new to the fraternity, guidelines make you learn and prevent from doing any clinical misadventure. After years of practice, our clinical knowledge helps us to conduct researches and guidelines are being updated. So both of them are essential for betterment of the patient.
A reason mentioned for emergence of MDR TB in India is not adhering to national treatment protocols.
Thus, I believe guidelines are necessary , but yes they should allow all the available and effective options to enable us the benefits of latest developments and provide individualized personalized care.
