Teachers when they adapt the project based learning or instructional system design models can help them and their students cooperate, promote interaction, information sharing among themselves.
It's necessary to activate higher order thinking skills because synthesis (or creation) is the highest level, in terms of revised Bloom's taxonomy (Anderson's and Krathwohl's revision). Methods of assessment should be in the deepest possible harmony with methods of learning. When students are doing a project or some other product - they achievement could be measured in mixed form and teacher may evaluate the process and its' product also.

The teacher could provide guidelines that are not prescriptive which allows the students to be creative for as long as they are able to meet the goals or standards set for a task. Encourage the students to be creative. The teacher could also group the students according to multiple intelligences and do the task/project in differentiated way. Also, the teacher may include creativity in the rubric; just ensure that the descriptors are stated clearly.
The teacher should act only as facilitator where students are given the chance to express their creative ideas and outputs. A rubrics shall be used to assess students' creativity.
Teachers can certainly help students develop creativity and there are many types of creativity, as such it is almost impossible to assess student creativity - it is possible to assess how creative they are, or how creative their response(s) are to something I would suggest.
Creativity needs space, time and acceptance to unfold. For school, this means that students need to feel safe so that they dare to express their thoughts. This is sometimes particularly difficult for students because they do not know if they are right and how their answers are received in the class.
The best way to encourage students' creativity is to create moments for creativity. Occasions where children can ask an open question in the room without the pressure of judgement and time pressure.
