While I don't have personal experience with a small scale shake table, a lab at Purdue University (Intelligent Infrastructure Systems Laboratory, IISL) has one that they use for developing structural health monitoring techniques and instruments.
Also, there is a University Consortium on Instructional Shake Tables (UCIST) that may be able to provide you with some useful information.

There is a lab at Universidad del Valle, Cali, Colombia, Southamerica (Laboratorio de Ingeniería Sísmica y Estructural, Linse) that has medium and small shaking tables for testing. The small one is described in the following attached link.
