"citation is wrongly placed in index area"

Why does this problem occur in Mendely?

This is how I solved this problem:

1- pressing ALT+F9 keys

2- Select the refresh option from the references tab

3- Removing the grayed out reference (highlighted).

I did step 3 several times until after selecting the refresh option, I did not encounter any errors.
